For people who are:

  • 45-49 years old - Once only
  • 75 years old and over - Annual


Why should we do a health assessment?

A Health Assessment is a 60 minute comprehensive review of risk factors that may cause chronic disease. This screening tool is used to recognise any signs in cognitive and physical deterioration or the need for extra support.

Physical check

  • Vital signs checks (heart rate, oxygen level, respiratory rate, blood pressure)
  • General check up (Weight, height, BMI)
  • Perform an electrocardiogram test (ECG)

Medical check

  • Check your immunisation status: especially for influenza, tetanus, and Pneumonia
  • Review your medication/s and risk factors

Mental check

  • Check for any signs of dementia/ depression (Mini Mental State Examination)
